• Clinical Autonomy: Submission of an online intake form, completion of a blood panel, or booking a consultation does not guarantee a prescription or medical treatment plan. All therapeutic choices, dosing protocols, and titration schedules are subject strictly to the autonomous clinical judgment of Daniel Ferguson, FNP-C, based on individual safety screenings and objective diagnostic data.

• Geographic Licensing Restraints: Apex Life Clinic provides professional medical telehealth and clinical services exclusively to patients physically located within the states of Missouri and South Dakota. Services are strictly prohibited and unavailable to individuals outside of these two authorized jurisdictions.

• No Physician-Patient Relationship: The information contained on this website is for informational purposes only. A formal clinician-patient relationship is only established after a patient completes an official intake questionnaire, signs a telehealth informed consent document, and completes a direct medical evaluation with our licensed provider.

2. Pricing Transparency, Billing, & Subscription Cancellation

• Upfront Pricing: All financial costs associated with clinical intake fees, laboratory review panels, and monthly wellness subscription plans are clearly disclosed on our site prior to checkout and booking.

• Automated Billing: Ongoing care programs are billed as recurring monthly subscriptions, processed securely through our HIPAA-compliant merchant processing partner, Stripe, integrated inside our Electronic Health Record (EHR), Carepatron.

• Subscription Cancellation: You may cancel an ongoing monthly wellness or health coaching subscription by providing written notice to the clinic at least seven (7) business days prior to your next scheduled billing date.

• Refund Policy: Due to federal regulations regarding custom prescription formulations compounded by our state-licensed 503A pharmacy partners, all fulfilled and shipped medication orders are strictly non-returnable and non-refundable.

3. Cookies & Privacy Integration
